In addition to populating a searchable "lessons learned" database, it has been my experience that it is also valuable to assign ownership to individuals or departments to make sure that their process artifacts (eg installation guides, boilerplate contracts, quotation worksheets, etc) are explicitly updated with the lessons learned. This creates a supply-driven (artifacts) as well as demand-driven (search) balance to continual improvement.
